How to access aged care services
Just Better Care is a Commonwealth Government Approved Provider of aged care services. Each of our offices is owned and operated locally, and can assist you with accessing services in your home and within the local community.
How to access aged care services?
In-home aged care support services can assist you to remain living in your own home and continue to access the community - living as independently as possible.
The first step to receiving funded support is to book an individual assessment, by visiting the My Aged Care
website or calling on 1800 200 422, to find out if you are eligible for a government subsidy (funding) and what level of care you need.
Assessments are done by the Aged Care Assessment Team (ACAT), or Aged Care Assessment Service (ACAS) in Victoria. An ACAT or ACAS professional will visit you in your home to find out about your needs.
The assessment is an opportunity for you to share what is important to you and what support would make a difference in your life.
What is a Community Home Support Programme (CHSP)?
The Commonwealth Home Support Programme is a government subsidised program that provides support for older Australians when they need a small amount of help in their own home.
This entry level support offers a range of services from those providers in your community that hold CHSP government funding.
The services and support will focus on building on your strengths and abilities to help you remain living independently and safely at home.
What is a Home Care Package (HCP)?
A Home Care Package is a government funded program that provides you with the services, and assistance to coordinate these services, to meet your individual health and care needs. Just Better Care Support Advisors assist you to organise services such as personal care, transport, help around the home, post-hospital, Alzheimer's care and dementia support, allied health and clinical services. These services are paid for through your package and assist you to be as independent as possible.
All HCPs are now delivered on a Consumer Directed Care (CDC) basis, which gives you, your family and/or carer the ability to choose the services you want, through the provider you wish to access them from.
There are four levels of HCPs and your level will be determined by the amount of support you require. Each package level has a different amount of funding (subsidy) that is paid to the provider of your choice by the Australian Government to deliver your care and services.

Government subsidy programs can ask for a customer contribution however, a customer should never be denied access to support if they can not afford to pay. There are two types of fees that you can expect to pay for a HCP; a Basic Daily Fee and an Income-Tested Care Fee. Depending on your income, you may be asked to contribute more to the cost of your care.
