To access a Home Care Package simply follow these six steps.
1. Eligibility
To access a Home Care Package (HCP), you first need to contact My Aged Care. A professional from the Aged Care Assessment Team (Aged Care Assessment Services in Victoria) will assess your eligibility for a Home Care Package.
2. Approval
An assessor will be arranged to visit you in your home to assess your care needs and determine the level of support required. The assessor will approve you for a level of care (Level 1 – 4) and place you in the National Prioritisation Queue. You will be notified of the outcome.
3. Financial considerations
The Australian Government subsidises a range of aged care services in Australia. If personal circumstances allow, you may need to contribute to the cost of your care.
4. Package Assigned
Once you reach the top of the National Prioritisation Queue, you will receive a letter stating that you have been assigned a package. You then have 56 days to find your provider of choice to deliver your home care services.
5. Choosing a provider
You can select the provider you want to deliver your services and work with them to discuss your individual requirements. The best way to start is by researching different providers and speaking to them directly about the services they offer. You can also utilise the My Aged Care Service Finder.
6. Service commencement
Once you have accepted your Home Care Package and signed an agreement with your service provider, your services can commence. If your needs change over time, a reassessment may be required to access a higher level of care.
