Refer a patient for In-Home Support Services

We welcome referrals from all healthcare professionals, case managers, and support coordinators seeking reliable in-home and community support for their clients or patients.

Just Better Care provides personalised care and support services for people living with disability, recovering from injury, or requiring aged care assistance. Referrals are accepted for customers funded through:

  • NDIS (National Disability Insurance Scheme)

  • TAC (Transport Accident Commission)

  • WorkCover and icare Attendant Care Programs

  • My Aged Care (Home Care Packages & Support at Home)

  • Hospital at Home programs

  • Private self-funded care

Some of our local offices also offer services under the Commonwealth Home Support Programme (CHSP).

Our services are flexible and tailored — available for short-term or long-term support, and scheduled as frequently or as occasionally as needed.
