Anyone supporting a person who may benefit from in-home support can submit a referral. This includes hospital discharge planners, General Practitioners, allied health professionals, support coordinators, case managers, community organisations, family members, and clients themselves.
Clients can be referred for a range of supports, including in-home aged care services, Support at Home services, disability support services, private in-home care, short-term support following hospital discharge, and ongoing personal care or domestic assistance.
Once a referral is received, the local Just Better Care team will review the information and contact the client, or their representative, to discuss their needs, eligibility, and support options. If appropriate, the team will arrange an initial consultation and begin planning services.
Response times vary depending on the client’s needs and location, but referrals are typically reviewed promptly and the client or referrer is contacted by the local office as soon as possible. If the referral is urgent, please indicate this in the referral form so the team can prioritise the request.
In some cases, services may begin quickly, particularly for private care or where urgent support is needed following hospital discharge. If the client is waiting for an aged care assessment or government-funded support, private in-home care services may be available in the meantime.
To help the team respond efficiently, include client contact details, key support needs, relevant health or mobility considerations, current services or funding, if applicable, and any urgency or discharge timelines. Providing as much detail as possible helps the team coordinate support more effectively.
