Post Hospital support

This service gives you the opportunity to continue your recovery in the comfort and familiar surroundings of home, while still receiving a high standard of clinical care.

We can provide a range of supports to assist you and your family after a hospital visit. This may include providing specialist clinical care by a registered nurse, assistance with administering medications, treating wounds, personal care, shopping, or meal preparation.

Our Post Hospital supports can include:

• Liaising with hospital discharge planners in organising your transition home.
• Understanding your illness and liaising with the hospital staff regarding your specific recovery plan.
• Medication management.
• Assisting with all your Domestic and Personal Care needs.
• Assisting with follow up appointments and liaising with your General Practitioner and other allied health providers.
• Complex wound management.
• Clinical assessments.
• Continence management.
• Catheter care.
• Providing injections.
