What Is an ACAT Assessment?
An ACAT assessment (Aged Care Assessment Team) is arranged through My Aged Care. It helps determine what government-funded aged care supports you may be eligible for, and what level of help you need to live safely and well at home.
Most assessments take place in your home, and an assessment organisation will contact you after you apply to arrange a time.

Why It Matters for Support at Home?
Support at Home is built around your individual needs and goals. The ACAT assessment helps confirm the type of supports that will help you stay safe, maintain independence, and get the right level of support now — with options to adjust if your needs change.
How to Apply?
You can apply for an assessment through My Aged Care in three ways:
- Online: Apply online using your Medicare card.
- By phone: Call My Aged Care on 1800 200 422.
- In person: Book an appointment with a Services Australia Aged Care Specialist Officer.

How to Prepare?
A little preparation can help you feel more comfortable and make sure the assessor understands your situation clearly.
- Have your Medicare card, ID and GP details ready
- List medications and key health conditions
- Note tasks that are becoming difficult
- Write down questions or concerns

What to Expect During the Assessment?
Most assessments are completed in your home. The assessor will talk with you about your daily routine, your health, your support network, and what services would help you remain safe and independent.
You can have a family member, friend, carer or registered supporter with you during the assessment.
What Happens After the Assessment?
After the assessment, you’ll receive a Notice of Decision and a support plan outlining the outcome and recommended supports. You can then take the next steps to start services with an approved provider.
