Short-term pathways for care and independence

Short-term pathways under the Support at Home program provide time-limited, targeted support to help older people remain safe, independent and comfortable at home.

These pathways are designed to respond to specific situations — such as recovery after illness or hospitalisation, restoring confidence and function, accessing assistive technology or home modifications, or receiving compassionate end-of-life care.

Within these short-term pathways, Just Better Care works closely with participants, families and health professionals to coordinate tailored support that aligns with assessed needs and goals. This may include intensive restorative care, access to assistive technology or home modifications, or additional in-home support during end-of-life care — all delivered with a strong focus on dignity, comfort and quality of life.

Where appropriate, short-term pathway supports can complement ongoing Support at Home services, helping people transition smoothly between care needs while continuing to live at home for as long as possible.

How to access Short-Term Support?

Short-term pathways are available as an additional funding tier and are provided as per your assessed need from an assessment with My Aged Care.

If you’re already receiving aged care services from Support at Home, talk to your Just Better Care local teamor care coordinator about whether a short-term pathway may benefit you.

In urgent situations or while awaiting a formal assessment, Just Better Care can provide private support for additional services or as top-ups to your existing support to ensure you or your loved one are not left without support.
