Short-term pathways for care and independence
Short-term pathways under the Support at Home program provide time-limited, targeted support to help older people remain safe, independent and comfortable at home.
These pathways are designed to respond to specific situations — such as recovery after illness or hospitalisation, restoring confidence and function, accessing assistive technology or home modifications, or receiving compassionate end-of-life care.
Within these short-term pathways, Just Better Care works closely with participants, families and health professionals to coordinate tailored support that aligns with assessed needs and goals. This may include intensive restorative care, access to assistive technology or home modifications, or additional in-home support during end-of-life care — all delivered with a strong focus on dignity, comfort and quality of life.
Where appropriate, short-term pathway supports can complement ongoing Support at Home services, helping people transition smoothly between care needs while continuing to live at home for as long as possible.
