What is Care Management?

Care Management is a key part of the Support at Home program. It ensures that your services are well-coordinated, responsive, and aligned with your preferences.

Your Care Manager will:

• Develop and regularly review your individual Care Plan
• Work with you to identify your goals and adjust services as needed
• Monitor your health, wellbeing, and safety
• Coordinate assessments and services from third-party providers
• Support you through any changes in your condition or situation

Is Care Management Included in My Funding?

Yes. Support at Home funding includes Care Management. Everyone receiving funded services will have access to a level of Care Management that meets their needs. If your needs are more complex, you may receive a higher level of support.

Your local Just Better Care team of care managers can also help you access urgent support, specialist referrals, or help with transitions such as returning home from hospital.
