Short-term care pathways under the Support at Home program are designed to meet participants' needs with tailored, time-limited support. Whether recovering from illness, nearing end of life, or in need of home modifications to stay safe.

Within these short-term pathways, Just Better Care can support participants and their families with more intensive support services or access to modifications or technology that provide greater independence, comfort and dignity

Restorative Care Pathway

Restorative Care Pathway
Regain strength, confidence, and independence after a health setback or period of reduced mobility.

This pathway aims to reduce reliance on longer-term services by helping you get back to doing the things you enjoy.

Support may include:

  • Allied health and nursing support for up to 16 consecutive weeks
  • Programs to improve strength, balance and mobility
  • Dietitian consultations to support healthy, independent living
  • Minor home modifications, like grab rails and ramps
  • Access to assistive technology and mobility aids

Eligibility: Support at Home participants may access this pathway up to twice per 12-month period. Terms and eligibility conditions apply.

End of Life Care Pathway

End of Life Care Pathway
Compassionate, high-priority support for people in the final stages of life who wish to remain at home

This pathway provides access to a higher level of in-home aged care services for people with a life expectancy of three months or less, helping them remain in the comfort and familiarity of their own home for as long as possible.

Older people can be referred for a high-priority assessment to access this support

Eligible individuals may receive up to $25,000 in funding, which can be used over a 16-week period to access the care and support they need.

Assistive Technology & Home Modifications (AT-HM)

Access greater independence, safety and comfort at home

The new AT-HM Scheme will make it easier for participants to access the assistive technology and home modifications they need — without having to save up funds from their individual care budgets.

As part of the Support at Home program, eligible participants will be assessed for their assistive technology or home modification needs. 

At the time of assessment, eligible participants will be assigned a funding tier based on the level of support required.

This funding can also include the cost of allied health prescriptions and wraparound supports, ensuring the safe and effective use of the equipment or modifications provided.

The Assistive Technology and Home Modifications List (AT-HM List) details the approved products, equipment and home modifications available through the scheme.


How to access Short-Term Support?

Short-term pathways are available as an additional funding tier and are provided as per your assessed need from an assessment with My Aged Care.

If you’re already receiving aged care services from Support at Home, talk to your Just Better Care local team or care coordinator about whether a short-term pathway may benefit you.

In urgent situations or while awaiting a formal assessment, Just Better Care can provide private support for additional services or as top-ups to your existing support to ensure you or your loved one are not left without support.
