It's all about you

A Care Plan is far more than just a list of tasks and a schedule. It is a personalised document that outlines the specific care needs, preferences and goals of the older person. It is created in collaboration with them and their family members or advocates.

It's reassuring to know the Care Plan is explained to all parties in plain English, with interpreter support available if required. This is a legislated requirement and an important step that Just Better Care takes pride in delivering with clarity.

Good to know

While funding used to be provided all in one pool, budgets for each category of support are now pre-defined under Support at Home. Just Better Care is experienced in working within this framework to develop your Care Plan to meet your needs and goals, while remaining within the boundaries of what's already been approved.

This rigorous approach ensures that the care provided is tailored, coordinated and, budget permitting, adaptable as needs may change over time. Ultimately, it helps to align the care expectations of all parties — participants, Support Workers, families and My Aged Care.

What to expect from the first visit

While every first visit is unique to the individual, our priority is to listen and plan. At Just Better Care we don't follow a rigid protocol, so we take our cues from you. In-home support services can only commence once your Care Plan and individualised budget are in place.

Initial visit with your Care Partner. A Care Partner will meet with you and your family members or advocates to sign the Service Agreement and create your Care Plan. Once this has been agreed, services can commence.

Meet your support team. We will talk to you about your regular Just Better Care team members who will be visiting your home to deliver the services. Continuity of care is important to us.

Services begin. Your Care Plan is live and your support routine takes shape. We regularly review your goals and can adjust services as your needs evolve over time.

Ways to fund your aged care support
Support at Home

Government-funded in-home aged care program for eligible older Australians. Includes Clinical, Independence and Everyday Living support categories.

Commonwealth Home Support Programme (CHSP)

Entry-level government support for older Australians who need a small amount of assistance to stay living independently at home.

Department of Veterans' Affairs (DVA)

Home care support available to eligible Australian veterans and war widows/widowers.

Private Home Care

Paid directly by the customer. No waiting, no government assessment required. Many customers also use private services to top up their government-funded support.

A foundation of understanding

The core of any effective Care Plan is ensuring it meets the needs of the participant's Support Plan, provided by the independent assessor. Once approved, you can choose your preferred local in-home support provider such as Just Better Care.

While every provider has a different process, we focus on a holistic evaluation and understanding of your physical health, mobility, cognitive function and emotional wellbeing. When we first start working with you, we also complete risk assessments and build pre-planning documents — ensuring we respect and integrate your cultural, spiritual, and personal preferences into everything we do.

Setting goals that matter to you

A vital part of your Care Plan is about setting your priorities. Every Care Plan should accurately reflect what matters most to you and your family. For some, the goal is maintaining independence at home, while for others it's about increasing social connection, improving mobility or managing chronic conditions.

We ensure your goals are realistic, achievable and flexible enough to evolve with you over time. As your care partner, Just Better Care is responsible for regularly reviewing your goals — and a Support Plan Review can be requested if your needs change substantially.

It's worth remembering you can pay directly for private services whilst waiting for a reassessment, so your care doesn't have to pause while the process takes place.

Support at Home — the three support categories
Clinical — fully government funded

Nursing, allied health and other clinical services. No co-contribution required from the participant.

Independence — co-contribution up to 85%

Personal care, social support and transport. Co-contribution is based on income and assets and determined during assessment.

Everyday Living — co-contribution up to 85%

Domestic assistance and meal preparation. Co-contribution is based on income and assets and determined during assessment.

Family members to support your decisions

As a family member of an older Australian starting a care plan, it's important to understand that you too play a key role in the care journey. By ensuring your thoughts and feedback are included in the planning process, the older person is more likely to feel better about the transparency of the process and the continuity of their care.

How do I get started with a Care Plan?

Start with My Aged Care to review your eligibility and book an assessment. Once you receive your Notice of Decision, simply choose Just Better Care as your provider and we will take care of the rest — including creating your personalised Care Plan before services commence.

Can my Care Plan change over time?

Yes. Your Care Plan is designed to evolve with you. Just Better Care regularly reviews your goals and services. If your needs change substantially, a Support Plan Review (reassessment) can be requested through My Aged Care. In the meantime, private services are available to bridge any gaps.

What is the difference between a Support Plan and a Care Plan?

A Support Plan is created by the independent assessor from My Aged Care and outlines the types of support you are eligible to receive. A Care Plan is created by your chosen provider — in this case Just Better Care — and translates that Support Plan into a specific, personalised schedule of services tailored to your daily life and goals.

Can I receive both government-funded and private services?

Yes. Many Just Better Care customers choose to top up their government-funded support with additional private services. This is a great way to access more hours, different services, or faster support without waiting for a reassessment.

If you are already approved for Support at Home, choosing Just Better Care is as simple as contacting your local office. Our team will guide you through the next steps, including setting up your Service Agreement and Care Plan so services can begin as quickly as possible.
